We offer many payment options
Payment is expected at the time of your visit. We accept cash, checks, money orders, VISA, MasterCard, American Express, and Discover Card. We also accept many private health insurance plans; check with your plan to see if you have coverage for our services. If so, bring your insurance card with you to your appointment. If your insurance does not cover our services, we can help you determine whether you qualify for other payment assistance; you will need to show some form of photo ID and know your Social Security number to apply. Women ages 19-44 may qualify for the state's Plan First! familiy planning program. If you have Medicaid, bring your card with you; almost all, if not all, services are covered.
Preparing for your appointment
If you are a new patient, you can save time at your first appointment by printing several forms, available in both English and Spanish, from our website and filling them out in advance. These include:
- Initial Medical History
- Funding Application
- Patient Rights, Responsibilities, & Billing Policies
If you are an established patient returning for your annual exam, you will need to fill out the Annual History form.
